Case Management Services - County of San Diego
The County of San Diego offers case management services designed to provide continuity of care within the mental health system. Case managers provide psychosocial rehabilitation intervention and resource management in order to assist individuals to obtain optimum independence. Services are delivered through a variety of programs to adults living with serious psychiatric disabilities who meet eligibility criteria and are indigent or have Medi-Cal.
